Health care records
Fees for supply of medical records required for ongoing health care are prescribed by State Government Legislation (Victoria). A quotation will be provided prior to supply.
About this service
Patients, or their authorised representatives, may request copies of their medical records, past investigation results, or formal medical reports from this practice.
A signed consent form must accompany all requests. A printable consent form is available to download below. Please print, complete by hand, scan or photograph, and attach it to your request submission.
We will provide a quotation before supplying any documents or information. Please read the fee information below before submitting your request.

Insurance & legal purposes
Where records or information are sought for insurance or legal purposes, statutory fee limits do not apply. Fees are set by the practice and will be quoted in advance.
Medical reports
Fees for the preparation of formal medical reports are not generally prescribed and are variable depending on the nature and complexity of the report. A quotation will be provided.
