DASS-42 Symptom Screening Questionnaire
This questionnaire asks about feelings and physical symptoms you may have experienced over the past week. It's used by Dr Daly as part of a broader clinical picture — it isn't a diagnosis on its own, and your individual answers are reviewed by the clinic rather than scored for you here.
There are 42 short statements. For each one, choose the option that best describes how much it applied to you over the last seven days. There are no right or wrong answers, and it shouldn't take more than 10 minutes.
The results will be sent to Dr Daly and provided to you at your appointment
